Enhanced Care Management
Enhanced Care Management (ECM) is a service model designed to provide comprehensive, coordinated, and client-centered care for individuals with complex health and social needs.
This approach aims to improve health outcomes by addressing both medical factors and the social determinants of health that contribute to a person's overall well-being.
Individuals will receive support with:
Medical care coordination
Finding doctors, scheduling appointments, managing medications, and setting up transportation
Applying for other community-based services such as housing deposits, medically tailored meals, asthma remediation, and day habilitation
Communication and coordination with entire care team
Health education and advocacy
